2010 VFC Recertification was due January 15, 2010. If your site has NOT yet submitted this year's recertification, VFC will not be able to process any vaccine requests.
You may complete and submit your 2010 VFC Provider Recertification electronically in order to continue participation in the VFC program during 2010, and prevent any delay in the receipt of your vaccine request. It is a federal requirement that each enrolled site receiving VFC supplied vaccines completes and submits Provider Recertification forms annually.
For your convenience, we have pre-populated most fields with your practice information and with VFC patient estimates based on data submitted by your clinic on the preceding year (when applicable). You will be prompted to review the information provided and update or revise it if necessary.
Important! Although the process may only take a few minutes to complete, you will only be able to print and submit completed forms, so make sure you set some time aside when you are ready!
